New Patients

This surgery does accept New Patients.  All new patients are required to complete a new patient details form.  Please bring your current Medicare Card and any Government Concession Cards with you.  We ask that you arrive 5 to 10 minutes early for your scheduled appointment to complete your new patient consent form.  If you fail to bring your Medicare Card with you a Private Consultation Fee will be charged.

New Born

If you have not yet registered your new born on your first visit we will hold the account.  Any subsequent visits a private consultation fee will be charged. 

Update your details

Please make sure your update your latest contact details (ie phone number and address) with Reception in case we need to contact you for any reason.  Keep on eye on your Medicare Card expiry date, and also your Government Concession Cards.

Late Arrivals

Late arrivals of more than 10 minutes for your scheduled appointment without giving notice, your appointment may need to be rescheduled, to avoid delaying the doctors with other patients.

Walk In Appointments

These appointments will be seen.  ***Fees may apply to all walk in consultations***.

Scripts

Scripts will only be written at a consultation.  Please do not wait until the last minute:

.  Check all scripts and repeats prior to your appointment

.  Book an appointment for a new script as soon as you fill your last repeat

SMS Recall System

We sometimes recall patients via a SMS Message.  We would appreciate a timely response to these messages. 

Please ensure that you update your mobile phone number with Reception.

Home Visits

A patient can arrange for a home visit if you meet the following criteria:

 
.     You are a regular patient of this practice
.     You live within a 10km radius of the practice
.     Where it is deemed by the GP to be safe and reasonable
.     The type of problem necessitates a home visit. 
 
In Emergency situation to to your nearest hospital.  For After Hours you can call "Dial A Doctor" on 1300 980 980.  
 

Lost Documents

A $15 fee will apply for reprinting of test request forms and referrals if the original has been misplaced or forgotten for your appointment with other providers.

Test Results

You are required to make a follow up appointment to obtain test results, (ie pathology/x-rays/ultrasounds etc) with your Doctor.  Test Results will NOT be given over the phone.  An administration fee will be charged for printing of any results.

After Hours Service

We use "Dial A Doctor" for any Medical Emergencies when this Surgery is Closed.  Contact them on 1300 980 980.  These visits are BULK BILLED.

Telephone Access to Doctors

Except in Emergencies Doctors will NOT be interrupted during consultations.  A message will be taken by staff and a Doctor will contact you at their earliest convenience.  Please assist us by giving as much information as possible, and an up to date contact number.  Most problems are best dealt with in a consultation.

Specialist and Other Service Providers

There may be fees associated with visits to other Health Providers (including pathology, specialists, radiology) referred by your Doctor.  It is recommended that you check the charges when making the appointment.

Privacy Act

A copy of our Privacy Policy is on display at Reception.  Your medical record is a confidential document.  It is the policy of this practice to maintain security of personal health information at all times, your record will only be available to authorised members of staff, and will not be released to any third party without your consent.

If you have a complaint, or any feedback (positive or negative) regarding our service, please feel free to ask to speak with the practice manager or your doctor.
